Forms for Communications Requests

• Communications Equipment Assignment Request in Word - use this form to request additional equipment to be issued to an individual or unit.  Units may redistribute radio equipment within their own units and this form is not required.  However in the event that the unit desires to request ORWG provide additional equipment for the assignment to an individual member of their unit, then this form is required to accompany the request.  Requests for additional equipment to be issued to a unit are to be sent to the ORWG Director of Communications, David Rudawitz Communications Equipment Assignment Request in pdf
• ORWG Station Authorization Request - use this form to request the issuance of a CAP radio station license to an individual or location.  Completed forms with electronic signatures should be emailed as an attachment to an email requesting a station license to ORWG/HQ at orwghq@earthlink.net attention "communications" or "DC".  The communications licensing officer, Maj Doug Richards, will then retrieve the application, process it and issue a license to the member.
